Billed annually.

This is a discount dental plan, not dental insurance.
Membership is non-transferable and non-refundable.
Membership fees must be paid in full at the time of enrollment.
The plan is valid for 12 months from the date of enrollment.
Discounts may not be combined with dental insurance, other promotions, or third-party financing offers.
Services not used within the membership period will be forfeited; no carryover or refund will apply.
Membership is only valid at Mint Dental Care and cannot be used at any other dental office or referred specialists.
The membership is the patient’s responsibility; it is not a qualified health plan under the Affordable Care Act.
